Investigators say a southbound Ford Focus, driven by a 34-year-old woman from Campbellsport, crossed over into the northbound lane of Highway 144 just south of Highway H.
The Campbellsport woman suffered fatal injuries and was pronounced dead at the scene by the Washington County Medical Examiner's Office. She was the only occupant in the car.
Deputies do not believed she was wearing a seatbelt.
The northbound vehicle was being driven by a 58-year-old Town of Farmington man. His 60-year-old wife was in the front passenger’s seat of this vehicle.
They were both trapped and had to be extricated from the wreckage. They were both treated at the scene by emergency personnel from Boltonville, Kewaskum and West Bend.
A Flight for Life helicopter took the male driver to Froedtert Hospital in Wauwatosa.

His wife was taken to St. Joseph's Hospital in the Town of Polk. She was pronounced dead at the hospital.
Both the husband and wife were both wearing their seat belts.
